Dementia Services Survey

Bury MBC have commissioned HKD Research to look into the dementia friendly groups which run in  Bury. If you attend any groups you will no doubt have been asked to complete a survey form. We have been asked by HKD Research to forward the following and hope that as many people as possible will complete the survey.

Survey about Dementia Friendly Community Support Services in Bury HKD Research is doing research for Bury Council to learn more about the dementia friendly community support services in Bury.  As part of this work, we hope you might fill in a quick survey about your experiences with community groups you attend.
